Join myLaurel as a Clinical Liaison in Brooklyn, NY. We are seeking a mission-driven Registered Nurse (RN), Social Worker (LMSW/LCSW), or Paramedic to act as the primary bridge between hospital leadership and our innovative acute care-at-home program. In this high-impact role, you will drive Acute Care Management by identifying eligible patients and fostering relationships with Interdisciplinary Teams.

- Salary: $100,000 – $105,000 base salary + performance bonus.

- Referral Growth: Proactively identify and develop new referral sources within the Brooklyn hospital system to expand our service line.
- Utilization Review: Conduct high-acuity Clinical Chart Reviews to screen patients for eligibility and facilitate the enrollment process.
- Patient Advocacy: Educate patients and families on the benefits of acute care-at-home and provide seamless Discharge Planning support.
- Operational Integration: Collaborate with hospital staff and the myLaurel clinical team to ensure accurate EHR Documentation and patient handoffs.
- Active Licensure: Must hold a current, unrestricted New York State license (RN, LMSW/LCSW, or Paramedic).
- Clinical Background: 4+ years of clinical experience in an Inpatient or Emergency Room (ER) setting.
- Technical Savvy: Proficiency in Electronic Health Records (EHR) and Google Workspace (Docs, Sheets, Slides).
- Adaptability: Ability to work flexible 8-hour shifts (e.g., 9:00 am – 5:30 pm or 11:00 am – 7:30 pm) to meet business needs.
- Connector Mindset: Excellent communication skills with a knack for business development and relationship management.
