Summary Description:

We are hiring passionate Physician Assistants and Nurse Practitioners to provide integrated medical services in patients' homes. THIS IS AN IN-PERSON POSITION in Poughkeepsie, NY.

You will travel to patients in a myLaurel vehicle equipped with diagnostics, procedures, and medications to deliver high-quality, in-home care.

Key Responsibilities:

  • Perform problem-focused assessments and health screenings.
  • Order, perform, and analyze diagnostic tests (e.g., labs, EKGs, POCUS).
  • Provide therapeutic interventions (e.g., IV fluids, wound care, splinting).
  • Coordinate mobile testing and follow-ups.
  • Integrate patient goals into care plans, including palliative principles.
  • Educate patients and families on managing health.
  • Communicate with referring clinicians and close the loop post-visit.
  • Support clinical guidelines, pathways, and quality initiatives.

  • Willing to learn procedures like IV placement, Foley care, GT care, and central line care.

Requirements

  • Current State Licensure as a PA or NP in New York.
  • Minimum two years of experience in relevant areas (e.g., ED, Urgent Care, Hospitalist).
  • Current BLS certification (ACLS preferred).
  • Valid driver’s license with a clean record.
  • Physical ability to navigate stairs and complete patient care in home settings.
  • Eligibility to work in the U.S. without sponsorship.

Special Considerations

  • Comfortable providing unplanned care in home settings.
  • Flexible scheduling, including weekends and holidays.
  • Full-time: 36-40 hours/week; 8- or 10-hour shifts (occasional after-hours visits).
